Service Description

Park Place Outreach will work with all eligible youth and young adults needing outreach services but we will target 14 -17 years old who are homeless or at risk of homelessness as well as those 18 -21 years old who age out of the foster care system and other systems. With the 18-21 year olds, Park Place Outreach does not provide shelter but we refer them to other agencies that provide emergency housing. An intake assessment is completed to gather information about client and their needs. Case management services are offered to all youth and young adults. In addition to case management, clients are provided with therapy, adult mentors, peer support groups, employment and educational support, health management, as well as emergency aid survival packs.
