Service Description

Provides services to pregnant and parenting teens. Offers individual counseling, childbirth education, parenting education using the Nurturing Parenting program, breastfeeding education, wrap-around case management, and support groups. Home visits are done bi-weekly for a period of up to six months.

Eligibility

Individuals 19 years old and younger (or up to 22 years of age if disabled). Teen must live with their baby (2 years old or under) and be the primary caregiver for their child.
