Provides case coordination (which includes ensuring access to the most appropriate behavioral therapeutic health services), teaching independent living skills, coaching, and assisting youth with acquiring daily living skills. Offers activities such as conflict resolution, money management, transportation, health and medical, nutrition and hygiene, academic and career options.

Eligibility
Individuals ages 15 to 22 with significant challenges in transitioning to adulthood due to insufficient familial support such as a family history of persistent conflict, rejection or abuse/neglect; substance use or other behavioral issues; disengagement from educational, vocational, or employment activities; family member(s) in the criminal justice system; a history of running away; or delinquency or antisocial peers.
