Service Description
Provides case management services and a small financial subsidy to approved caregivers providing in-home care. Subsidy payments, though limited in amount, are intended to help offset the cost of housing, food, clothing, and incidentals and expenses related to medical, pharmaceutical, and dental services not covered by Medicare, Medicaid, or another form of insurance.
