Individuals ages 60 and over with Medicaid who have scheduled a COVID-19 Vaccine.

If you are in a health plan, call your plan provider to get the name and number of the transportation service. If you are not on a health plan, call 1-877-254-1055. The following information needs to be provided during the phone call: full name, Medicaid ID, address of pick up, date of birth, name of health plan if enrolled, date and time of appointment, and phone number.
