Service Description

Provides pregnancy help, support, and guidance from nurses who make home visits to first-time moms. Each mother is partnered with a registered nurse early in pregnancy. The mother then receives ongoing visits through her child's 2nd birthday.

Eligibility

Individuals pregnant with their first child must meet low-income requirements, be less than 28 weeks pregnant; receive Medicaid, WIC, or SNAP (food program); and live within the following zip codes: 33064, 33069, 33060, 33319, 33313, 33311, 33024, 33025 or 33023.

Application Instructions
